Psychiatry Forensic APP (Job ID: 11316)Job SummaryUnder minimal supervision, provides professional psychiatric midlevel care. Provides direct patient care in accordance with the statutory limitations for Midlevel Practitioners and may perform administrative support functions as required.

Responsibilities
Essential Duties and Responsibilities
1. Evaluates emotional components, the impact of health related issues and assists clients to identify and meet treatment goals.
2. Anticipates and identifies issues and initiates resolutions for the corrections mental health patient population.
3. Monitors and evaluates the psychiatric condition of the patient and in collaboration with the psychiatrist and other disciplines and standards of care or applying appropriate protocols, make decisions regarding treatment changes.
4. Analyzes the assessment data in order to formulate a unique plan of care through identified needs.
5. Competent in addressing psychiatric needs ranging from inpatient to outpatient levels of care
6. Demonstrates and understanding of assessment findings as they relate to disease process and continuum of care; relates findings to multidisciplinary team.
7. Integrates research behaviors into clinical practice by demonstrating awareness of research findings, identifying areas of practice that can be improved through research utilization and assisting in research
8. Collects data to promote self, unit and hospital improvement.
9. Identifies and reports any abnormalities in assessment data.
10. Performs data best practices and HIPAA compliance.
11. Prescribes appropriate medications in accordance with pharmaceutical guidelines and standards of practice.
12. Demonstrates competence in the prescribing of psychotropic medications.
13. Performs documentation audits and develops any new documentation required to meet the requirements of the regulatory agencies.
14. Documents initial admission/shift assessments per unit-specific guideline of care. Ensures daily goals and plan of care transcribed and complete.
15. Ensures documentation is complete, concise and reflects care given.
16. Documents completely complying with policy and procedures in regards to hospital policies and procedures.
17. Adheres to the Denver Health principals of Informing Clinical Practice, Interconnecting Stakeholders, Improving Population Health, and Personalizing Care.
18. Writes orders, and evaluates patient’s diagnostic and therapeutic services.
19. Refers patients to specialized services as needed.
20. Orders initial and/or routine laboratory tests and follow-up on results, orders additional tests or initiates referral appointments with specialized caregivers.
21. Initiates a unique, outcome focused plan of care based on current nursing practice with measurable outcomes and goals for the patient/family.
22. Evaluates, modifies and provides safe drug dosage as necessary and collaborates with the MD to assure safe sedation and monitoring tailored to the individual patients need.
23. Collaborates with other health care professionals in assessing, planning, implementing and evaluating care for individuals.
24. Collaborates with Risk Management to resolve identified and potential problems (i.e. Sentinel events) and ensures proper reporting of events as dictated by policy.
25. Manages department changes in terms of communication, minimized work disruption and attention to employee morale issues.
26. Collaborates with experienced investigator on a research project or develops a research project.
27. Coordinates care with multidisciplinary team.
28. Leads an environment that empowers staff to participate actively in setting goals, defining programs, and evaluating outcomes.
29. Participates actively in employee and patient rounding per unit expectations.
30. Practices sound safety and infection control practices according to agency policies and procedures; Identifies safety and infection control issues.
31. Evaluates quality, effectiveness, and efficiency of nursing care provided.
32. Promotes quality care by redefining priorities for care based on evaluation of patient/family response or changing condition.
33. Evaluates practice in relation to professional standards, statues, regulations, hospital and departmental policies, and standards of care.
34. Assumes accountability for achieving goals set jointly with the manager and educator for the next evaluation period.
35. Performs quality control on appropriate systems and equipment.
36. Delivers care in a nonjudgmental, nondiscriminatory manner that preserves patients’ rights.
37. Acts as a patient advocate.
38. Assures patient privacy before, during and after procedures.
39. Performs other duties as assigned.
Knowledge, Skills & Abilities1. Is knowledgeable and compliant with all regulatory standards, including specialty (i.e. Corrections) level requirements
2. Knowledge of major concepts, principles and theories related to drug, alcohol and psychiatric disease processes.
3. Knowledge of individual and group dynamics and treatment methods to assist client in achieving goals.
4. Skill in preparing written reports, medical records, and other documentation.
5. Ability to be involved actively in multidisciplinary teams relevant to quality improvement.
6. Ability to serve as a consultant to resolve/discuss ethical issues.
7. Ability to use computers and computer systems.
8. Ability to Drive to different DH locations may be required in this position. If so, a valid driver’s license is required.

Computers and Technology
1. Knowledge of Microsoft Office
2. Knowledge of the EPIC Electronic Health Record System. Supervisory Responsibilities
This position has no supervisory responsibilities. Education Masters Degree in Psychiatric/Mental Health Experience
Experience Typically, one year experience in midlevel psychiatric practice. 2. Three years prior psychiatric experience strongly preferred
Credentials

1. Basic Life Support for Health Care Providers certification/CPR. 2. Possession of a National Certification for Advanced Practice Clinical Nurse Specialty in Mental Health. 3. Licensed by the Colorado Board of Nursing or Physicians with Prescriptive Authority. 4. Advance Practice Nurses without Provisional Prescriptive Authority must be actively working towards obtaining Provisional Prescriptive Authority within five years from date of hire. 5. Advance Practice Nurses with Provisional Prescriptive Authority must be actively working towards Full Prescriptive Authority within five years after provisional authority is granted. 6. DEA Buprenorphine X Waiver required upon hire. 7. All Certifications and Licenses required for this job must be kept current as a condition of continued employment
