This position is located at the Department of Mental Health Forensics Services, Crafts-Farrow Campus, 7901 Farrow Rd, Columbia, SC 29203.
As the Counselor II, you will be under the supervision of the Counselor III and/or Forensics Program Manager Clinical Treatment Services. Facilitates evidenced informed psychosocial and educational groups within a Forensic Psychosocial Rehabilitation Treatment Mall and Forensic Units serving adults with serious and persistent mental illness and is implements services and interventions as identified on a multi-discipline plan of care.
Responsibilities also include but are not limited to:
- Assists the Program Manager and Program Coordinator in planning, organizing and coordinating evidence informed Psychosocial rehabilitation and educational groups for Forensic patients participating in Treatment Mall programing and assigned unit programming.
- Leads groups daily as scheduled and maintain adequate materials and supplies as needed for group function.
- Documents in the Electronic Health Record progress notes and treatment goals/objectives and interventions on master Treatment Plan.
- Performs documentation of progress notes within twenty-four hours for individual and/or group sessions noting patients progress towards meeting treatment plan goals/objectives. Progress notes are professionally documented with accurate punctuation and grammar.
